Provider Self Review

Provider Reviews

Once per year, providers will be formally evaluated. Contracts with providers are re-signed in the months of February / March each year. Just prior to the signing of a new contract, providers will be evaluated using the government form as outlined in the Family Day Home Standards Manual (July 2011) Agency Standard 4. The assessment will include a review of the provider's history of compliance to standards, training and professional development, parent satisfaction, and history of complaints and incidents. The Agency will use the results of the performance assessment to determine if the provider should maintain their approval status. Provider review documents: “Provider Self Evaluation” to be signed with review, Provider Performance Assessment (Government Form), Provider Annual Evaluation (Agency Form), Provider Caregiver Interaction Skill Checklist (Agency Form)
